This form is to authorise MetLife Insurance Limited to debit premiums from your account with another financial
institution.
NOMINATION OF BENEFICIARY FORM (MULTIPLE)
To nominate multiple beneficiaries, you should use this form.
NOMINATION OF BENEFICIARY FORM (SINGLE)
Nominate your sole beneficiary with this form.
MEMORANDUM OF TRANSFER
A memorandum of transfer form is for use when the ownership of the policy is to change.
DEATH CLAIM FORM
From our MetLife family to yours, we would like to extend our most sincere condolences. Please use this form if you are making a death claim on a 'Term Life' policy purchased through Citibank, your broker or directly through MetLife Insurance.
CASH ASSIST CLAIM FORM
Complete this form if you have a Cash Assist insurance policy and are making a claim under your Cash Assist policy that you purchased through Citibank, your broker or directly through MetLife Insurance.
REST EASY PLAN CLAIM FORM
Complete this form if you have a Rest Easy Plan insurance policy and are making a claim under the Rest Easy Plan that you purchased through Citibank, your broker or directly through MetLife Insurance.
TERMINAL ILLNESS CLAIM FORM
Complete this form if you have a Term Life insurance policy and are making a terminal illness claim on the plan that you purchased through Citibank, your broker or directly from MetLife Insurance.
TERMINAL ILLNESS MEDICAL STATEMENT
A medical practitioner should complete this form if you hold a Term Life policy and obtained your insurance policy through Citibank, your broker or directly from MetLife Insurance and are required to provide a Terminal Illness Medical Statement while making a terminal illness claim.
TRAUMA INSURANCE CLAIM FORM
You should use this form if you are making a Life Care Trauma claim on a Life Care Trauma insurance policy that you purchased through Citibank, a broker or directly through MetLife Insurance.
MEDICAL EXAMINATION REPORT
This report is to be completed by the medical attendant.
APPLICATION FOR INSURANCE
Please use this insurance application form if your insurance was acquired through your employer and you are applying to become a member of a Corporate Group Insurance policy.
TELE-INTERVIEW PREP KIT
You should use this form if you purchased your insurance through you financial adviser and are preparing for a tele-interview for underwriting purposes.
NON-SMOKER DECLARATION FORM
Use this form when you want to alter your smoker status to non Smoker on your insurance policy
